I used a VPS from DirectSpace Networks, buget VPS hosting starts at $4/month. FranTech has some budget VPS that start at only $15/year when they are available that are also fun.- A phone number.
I used IPKall which provides free DIDs.
I am assuming that IPKall supports a maximum of a 2 hour phone call... because that's the longest anyone has called for.- Linux
CentOS seems to be the most supported for asterisk.- Asterisk

I created a script to setup a basic Asterisk/FreePBX server on a virgin CentOS install- Perl and the Asterisk::AGI module
I used cpan and ran
install Bundle::CPAN
install Asterisk::AGI
install DBD::mysql- the pi agi script that I wrote
- ... and pi!
